Purpose

This confidential questionnaire helps Phase Advance understand the asset, the development decision at hand, the available evidence, and the practical path to an engagement.

The information will be used solely to assess scientific and operational fit and to prepare a preliminary scope of work. Please provide best-current estimates; ranges are welcome. If any requested information is not yet available, confidential at this stage, or better discussed live, please indicate that.

Estimated completion time: 25–40 minutes per asset, excluding document upload.

Please complete: Sections 1 and 6–9 once per company; Sections 2–5 once for each asset.
